To Know About Labile Hypertension

Labile hypertension can be asymptomatic or can be accompanied by symptoms such as headache, palpitation or flushing. BP usually falls spontaneously without intervention; BP elevation is usually readily attributed to emotional stress, by both physician and patient.

Normal lability would be harmful in a patient with a certain medical condition (like chronic aortic dissection, Marfan syndrome, angina or cerebral aneurysm in which reduction at even normal liability could be proposed to be beneficial.

There are no quantitative criteria to define or diagnose labile hypertension or normal from abnormal lability of BP. Labile component of BP as a clinical dilemma Patient with labile hypertension experience transient but substantial increase in BP. The increase often but not always occurs in the setting of emotional distress particularly anxiety and are likely mediated by sympathetic activation. 

Eliminating hypertension is not possible but adequate control is often achievable. A regimen that antagonizes sympathetically mediated BP elevation like ( α/β blocker eg carvedilol etc)seems more logical than treatment with other agents. An Anxiolytic agent also helps in reducing in lability of hypertension. Newer devices are on the way which detects lability of hypertension, and sympathetic tone and regulates it accordingly.
